Patient Records & Transparency Fee Schedule
At My Best Health is committed to ensuring patients have seamless access to their health information in accordance with 2026 HIPAA Right of Access standards. Our fee structure is designed to be reasonable and cost-based, reflecting the actual labor and supplies required to fulfill your request.
Patient-Directed Requests
Electronic Health Records: A flat fee of $6.50 is applied for all records delivered via secure email or patient portal. This inclusive fee covers the digital preparation and secure transmission of your clinical data.
Inspection of Records: There is no charge for patients who wish to inspect their records in person at our Phoenix clinic.
Paper Copies: A fee of $0.25 per page applies, plus the actual cost of labor for copying and standard postage. In accordance with federal law, no search or retrieval fees are applied to patient-directed requests.
Third-Party & Legal Requests
Requests initiated by third parties, such as insurance carriers or legal counsel, are processed under Arizona state guidelines:
Administrative Handling: A flat $35.00 clerical fee is applied per request.
Reproduction Rate: Records are billed at $0.60 per page.
Shipping: Actual postage or courier costs will be added to the final invoice.
2026 Compliance Disclosures
2026 mandates regarding digital access and sensitive records:
Right of Access: Patients have the right to inspect their records and may take photographs of their Protected Health Information (PHI) during an in-person inspection.
Response Time: At My Best Health will respond to all record requests within 15 days of receipt.
Sensitive Records (42 CFR Part 2): Substance use disorder records receive heightened protection and will not be disclosed in legal proceedings without specific written consent or a court order.
Website & Digital Privacy
Data Collection: We collect names, emails, and phone numbers provided through our contact forms for the purpose of scheduling and patient communication.
Security: We utilize industry-standard encryption to protect information submitted through this website.
