Why Complete the Intake Form in Advance
Completing your intake information online before arriving streamlines your first appointment significantly. Rather than spending the first several minutes of your visit filling out paperwork, that time is instead spent with Dr. Dorchester or Dr. Sabal conducting your actual clinical evaluation — range of motion testing, neurological screening, and a thorough discussion of your symptoms and accident history.
For automobile accident patients, this is especially important. The information you provide helps our team establish an accurate timeline of your accident, document your symptoms as they existed at the time of your injury, and begin the forensic medical documentation process that protects both your health and, where applicable, your personal injury claim.
What Information You Will Need
To complete the intake form efficiently, please have the following information available:
Your accident details, including the date, time, and general circumstances of the incident.
A description of your current symptoms, including pain location, severity, and any numbness, tingling, or radiating discomfort.
Any prior treatment you have already received, including emergency room visits, urgent care, or other medical providers.
Your insurance information, including automobile insurance carrier details if applicable, and any attorney contact information if you are already working with legal counsel.
A list of current medications and any relevant medical history.
What Happens After You Submit
Once your intake form is received, our clinical team reviews your information before your scheduled appointment. This allows Dr. Dorchester to begin formulating an initial clinical impression and identify which diagnostic protocols — including CAD syndrome evaluation, AOMSI assessment, or mTBI screening — may be appropriate for your first visit.
If you have been in a recent automobile accident, we recommend completing this form as soon as possible. Evaluation within 72 hours of a motor vehicle collision is critical to establishing an accurate medical baseline and protecting the documentation timeline for any personal injury claim.
