Your first visit

Bring:

  • Photo ID

  • Auto insurance information, if you were in a collision

  • Health insurance card

  • Your attorney's contact information, if you have representation

  • Any imaging or records from an ER or urgent care visit

You can save time by completing the New Patient intake form before you arrive.

Most first visits run about [45–60] minutes. That includes a full history, examination, and — where appropriate — treatment the same day.

Costs and insurance

If your injury came from a collision that wasn't your fault, you may have no out-of-pocket cost. We accept auto insurance and treat on personal injury lien.
