Using the portal helps streamline your appointment so more of your visit time is spent with Dr. Dorchester or Dr. Sabal — not on administrative forms.

First Time Visiting?
If you are a new patient, please complete our Consultation Intake Form instead. The intake form gathers the clinical history needed for your first evaluation, including details about your accident or injury, current symptoms, and any prior treatment.
